The work setting is an urban hospital that is renowned for its specialized acute care services. The hospital caters to a diverse patient population with complex medical conditions. The facility operates 24/7 and handles a high volume of patients daily. Currently, interprofessional teams are present in the hospital, which is composed of doctors, nurses, pharmacists, physical therapists, social workers, nutritionists, and other allied health professionals. While the presence of interprofessional teams is valuable, there is still a need to enhance their communication and coordination to optimize patient care outcomes.
